For our physician colleagues:
- We are referral based. Referrals can be sent by:
- Faxing a consult note with clinical question and medical history to our clinic
- Completion of our referral form
- Patients are seen with shorter wait times usually within 2 weeks
- Please indicate if you are part of a FHO or FHG on the referral - no outside use penalties will be incurred for patients seen by us
- We do not see patients for suspicious ovarian cysts, endometrial polyps or colposcopy- please refer to surgical gynecology/colposcopy for these issues.
