Patient information:

  • To access a service, referral is required. 
  • Please provide the referral form to your family doctor or walk in doctor. If you do not have a family doctor, you are welcome to visit Maple Walk in clinic where our team will be happy to assist you with a referral. 
  • NO referral needed for: medical abortion (for up to 8 weeks of pregnancy), STI testing/treatment and early prenatal care- Please call our clinic to book appointment for these services
  • For pregnancies beyond 12 weeks, please ask your physician for a referral to an obstetrician. 
  • We see private patients and those covered by OHIP and IFHP. 
  • We currently offer services in english and hindi.
